• Worker Appeal of Claims Decision

  Use this form if you are a worker who wants to appeal a decision of the WCB about your claim - e.g. that your claim is not acceptable, that you are not entitled to wage loss benefits, etc.

• Employer Appeal of Claims Decision

  Use this form if you are an employer who wants to appeal a decision of the WCB about a claim made by one of your workers - e.g. that the claim is acceptable, that the worker is entitled to wage loss benefits, that you are not entitled to cost relief, etc.

• Appeal of Assessment Decision

  Use this form if you are appealing a decision of the WCB's Assessment Committee.

• Appeal of Victims' Rights Decision

  Use this form if you are a victim of crime who is appealing a decision of the Compensation for Victims of Crime Program.
